Each Player MUST have a current Sports Physical completed prior to being able to practice or play. (Physicals from last season can not be used for this season.)

Please print and use the 2025-2026 AIA Sports Physical forms below.  Fill out all player information before arriving to your appointment or any Sports Physical clinic.  

We plan to arrange a Sports Physical clinic for our players prior to the start of the season. Date and location to be determined. You may also choose to have it done with a provider or clinic of your choice.
